Speak Up Ontario Campaign

Speak Up Ontario is an initiative of Hospice Palliative Care Ontario to improve awareness of Health Care Consent and Advance Care Planning in the Province of Ontario. Ontario has unique requirements with respect to Health Care Consent and Advance Care Planning and this website focuses on providing information, tools, and resources specific to the Ontario legal context. Hospice Palliative Care Ontario’s Health Care Consent and Advance Care Planning Community of Practice provides leadership and guidance for the project. Speak Up Ontario is part of a larger initiative called Advance Care Planning in Canada – overseen by a National Advance Care Planning Task Group comprised of individuals representing a spectrum of disciplines, including health care, law, ethics, research and national non-profit organizations.

National Framework

The Advance Care Planning in Canada: A National Framework and Implementation Project was initiated in 2008. The long-term goal of the project is to raise the awareness of Canadians about the importance of advance care planning and to equip them with the tools they need to effectively engage in the process.
You may never need your advance care planning wishes – but if you do, you’ll be glad that you had the conversation.

The secondary goal is to prepare professionals/health care providers with the tools they need so they can facilitate and engage in the process of advance care planning with their clients.

The Project has developed a National Framework to guide ACP program and policy planning and implementation.
