Hospice Palliative Care Ontario has been designated in the OPCN 3-Year Action Plan (2017-2020) as the lead for Health Care Consent Advance Care Planning and Goals of Care (HCC ACP GoC) in Ontario. HPCO’s recent work with the Ministry of Health and Long-Term Care and ongoing Ontario Palliative Care Network (OPCN) partnership requires that we support HCC ACP GoC project and research initiatives in Ontario to ensure they are reflective of the Ontario Legal Framework. This includes working with core partners across all care settings and liaising with all researchers working in HCC ACP to identify and utilize compliant HCC ACP practices and resources in Ontario. This is considered a priority for all partners given the continued extent of non-compliance in Ontario regarding HCC ACP and GoC discussions.
Across the province considerable time and effort is often spent by associations, organizations and projects to develop HCC ACP related documents and processes. In an effort to support this work, the HCC ACP CoP Leadership Advisory Team has created the HCC ACP Ontario Tool Kit to ensure that the language and intent of all related initiatives in Ontario complies with the Legal Framework.

Health Care Consent and Advance Care Planning Community of Practice’s Resource Review service – Creating new resources? The Community of Practice offers a review of Health Care Consent and Advance Care Planning materials (i.e. content, policies, procedures and other materials) to ensure the language and intent complies with the Ontario Legal Framework. Vetted final resources can then be acknowledged by the CoP as being reflective of the Ontario Legal Framework, and with permission, shared on the Ontario Resources of the Speak Up website repository.
